Complete the following steps to open your account

Submit the following forms as necessary, (fill them in online and then print or print and complete manually (print or type) (requires Adobe Reader) be sure to print two copies: one for submission to IRA Services and one for your files:
- Account Application (Required, include a copy of a government issued photo identification [i.e. drivers license or passport])
- Beneficiary Designation (Complete this form to designate your beneficiaries)
- Representative Authorization (Complete this form if you want to allow your representative to have access to your account information)
- Transfer Authorization (Complete this form if you wish to transfer assets from another IRA custodian)
- Investment Authorization (Complete this form when you are ready to make your investment. This should accompany any paperwork required by the investment provider [i.e. subscription agreement])
- Internet Access Request (Complete this form if you would like online access to your account)
- Private, Family or Single Member LLC Agreement (Require to be completed if you will be investing in this type of asset)
Print the following documents for your files:
- Privacy Notice
- Custodial Agreement (Traditional, Roth or Simple, depending on the type of account you are opening.
- Fee Schedule and Financial Disclosure.
- IRS Publication 590 (is it very lengthy, you may want to save it to your PC or review it online.
- Internet Access Terms and Conditions (only if you request online access)
Prepare a check for your deposit and fees (establishment and annual account fees). Remember that you must maintain a minimum balance in your custodial cash account of $300.00. The fees and the minimum cash will be deducted from your gross check and the balance will be available for investment. If you are transferring assets, you need to prepare a check for the amount of fees and minimum cash.
Mail the completed forms and check to the following address:

The opening of your account may be delayed if you do not return the application, photo ID and fee check.
